under Act No. 101/2000 Sb., on the protection of personal data, and Regulation (EU) 2016/679 of the European Parliament and of the Council.
By sending this form and clicking on the option "I agree", I as a Data Subject give consent to the processing of my personal data specified below which I voluntarily provide to the Data Controller in the manner set out below in this consent. I acknowledge that these personal data concerning me constitute personal data as defined by Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC ("GDPR"). At the same time, I declare that I am at least 16 years of age, and if not, I confirm that this consent has been expressed or approved by a person having parental responsibility for me.
1. Controller
The Data Controller is Pavel Vraštil, registered office at Veselíčko 112, 75125 Veselíčko, ID: 73274704 (the "Controller").
2. Processing of personal data
2.1. I voluntarily provide the Controller with the following personal data: first name, last name, email, phone number.
2.2. I agree to the processing of my personal data listed above for the following purposes:

marketing purposes of the Controller associated with the offer of products and services; sending information about organized events, products, services and other activities (e.g. by sending newsletters, telemarketing); contacting for market research purposes and for marketing research; contacting for Christmas, Easter or other holiday wishes, and sending discount vouchers, gifts, etc.;
processing of personal data is necessary to fulfil these purposes.
2.3. The legal basis for data processing is Article 6(1)(a) of the GDPR.
2.4 The Controller processes personal data that are adequate, relevant and limited to what is necessary for the purposes for which they are processed.
2.5. In addition to the Controller, the following entities will also collect and process the personal data listed above:

Recipients of personal data: The Controller is entitled to transfer the personal data of the Data Subject in particular to the following recipients and categories of recipient: contractors and staff of the Controller, transport service providers, marketing and advertising service providers, financial institutions, state authorities in the fulfilment of statutory obligations under the relevant legislation, and other persons in various contractual relationships with the Controller.

2.6. The Controller is entitled to process the personal data listed above for 5 years from the giving of my consent. I acknowledge that before the period specified above expires, the Controller may contact me to renew my consent and that without my renewal, my personal data will be permanently deleted after the period specified above expires.
3. Rights of the Data Subject
3.1. I acknowledge that, as a Data Subject, I have the right to obtain from the Controller access to my personal data (Article 15 of the GDPR), their rectification or erasure (Article 16 or 17 of the GDPR), restriction of processing (Article 18 of the GDPR), and the right to object to the processing (Article 21 of the GDPR). Under the terms of Article 20 of the GDPR, I have the right to the portability of personal data that concern me and that I have given to the Controller as a Data Subject.
3.2. As a Data Subject, I can contact the Controller concerning the processing of my personal data by email ubytovani@tlustysvist.cz.
3.3 If I, as a data subject, believe that my personal data are being processed in violation of the law, I have the right to contact the Controller requesting remedy. If the request is found legitimate, the Controller will immediately remedy the unlawful situation. This is without prejudice to my ability as a data subject to file a complaint directly with the Office for Personal Data Protection.
3.4. I hereby give consent to the processing of my personal data by the Controller for the purposes, to the extent and for the time specified above. I acknowledge that the Controller will archive this consent for the purpose of fulfilling the legal obligation of the Controller to be able to substantiate that I have given my consent to the data processing.
